Case Presentation: Smile Makeover

What makes a beautiful smile? Is it the size of the teeth? The shape? Color? As this case demonstrates, the answer is all of the above! Careful details help us reconstruct a smile that is not only beautiful but appropriate for the patient's face shape, age and mouth size. The goal is to create natural appearing teeth while avoiding the "chiclet" look that can come from too little color or shape variation

For this patient, we used Emax lithium disilicate crowns to restore the cracked and decayed anterior teeth. Additionally, you can see where a bridge was used to replace the patient's missing canine on the photo right. As you study these pictures, take note of the subtle color gradient from the edges of the teeth to the gum line. This gradual darkening mimics natural teeth and creates a very convincing profile. Overall, we could not be happier with the results!

Posts and Cores for Dental Crowns

If you have had a dental crown recently placed (particularly on a root canal treated tooth) you may have been surprised to learn that you needed a post or core placed in addition to the crown. These procedures allow us to maximize the strength of the natural enamel and add structure back to broken down teeth. In every instance, they are an integral part of the long term success of a crown. 

When teeth have root canals placed or deep cavities removed, they are left with a large hole in the center of the tooth or a missing wall of enamel. For a crown to be successfully cemented, the tooth has to have a fairly regular "teepee" shape. Any large holes or divots in the tooth prevent the lab from making a successful restoration.  A core is a bulk of material added to the tooth to provide strength and structure prior to finishing the crown. They are made from a variety of material, depending on the specific needs of the tooth and the plan for the final crown. Many times the endodontist will place the buildup themselves once the root canal is finished. 

When a tooth is missing multiple walls of enamel or lacks enough structure above the gumline (as is common with fractured teeth), a post may need to be placed in addition to a core. Posts are solid metal or composite rods that are cemented in the roots of root canal treated teeth. They extend upward and act as an anchor for the core buildup. Without a post, large cores are prone to failure and fracture. This causes huge headaches, particularly after the root canal and crown are completed. An important point to note is that posts can only be placed on root canal treated teeth, as it needs to be placed within the root system. In rare instances, a root canal may be performed on a healthy tooth so that a post can be placed.

We always try our best to notify patients of the potential need for a post and/or core prior to starting crown treatment. These procedures are not optional steps, but rather necessary precautions for the long term stability of the tooth. Perfecting Incisors using Emax Crowns

One of the most common dental anomalies is underdeveloped lateral incisors. Often referred to as peg laterals, this condition typically presents as two lateral incisors that look smaller and mismatched when compared to the central incisors. For today's patient, Dr. Kari Ann Hong chose to correct the cosmetics of this smile using two beautiful Emax lithium disilicate crowns.

Since the incisors were already undersized, only a minimal amount of tooth structure had to be removed in fitting the crown. By working with Opus One Dental Lab a near perfect match was made to the natural central incisors. Notice the delicate translucency near the edges of the teeth and harmonious shape. We even captured the pronounced developmental grooves (called mamelons) on the central incisors. The result is a beautiful smile that looks more natural than the "before" shot!

Zirconia Crown and Bridge Replacement

Subtle changes can have powerful effects in cosmetic dentistry! Check out the crowns and bridge patient's before photos. The opaque shade and rounded features gave these teeth a definite "chiclet" appearance. Additionally, gum recession had left the teeth with dark lines surrounding their bases that contrasted with the white porcelain. By working with the Opus One Dental Laboratory, we were able to create new crowns with more lifelike shape and translucency. Even with similar color shades, the new crowns/bridge look infinitely more natural and beautiful in this patient's mouth.

Case Presentation: Temporary Veneer Update!

Time for a quick update! We previously reported on this patient's progress when they were still in temporary veneers. Now that the final products have been delivered, the dramatic transformation is complete. All four maxillary incisors were refinished with Emax lithium disilicate veneers that perfectly match the cosmetics of natural enamel. By treating all four teeth, we were able to create excellent symmetry and color matching. You could never tell any dental work was done! 

Veneers offer great cosmetic changes with minimal modification to the natural teeth. Since veneers only bond to one tooth surface, we only need to remove enough enamel to accommodate the new material. Dental Crowns In Thousand Oaks

Check out this new video testimonial from one of our very satisfied patients! Twenty years ago, Jennifer suffered from pain and headaches associated with stress on her temporomandibular joint (TMJ). In solving this, her previous dentist used dental crowns to improve the way her teeth came together and ease her TMJ pain. Although the procedure was successful, twenty years of wear combined with gum recession had affected the appearance and function of these restorations. 

Our main focus with this case was creating a long lasting solution that wouldn't bring back Jennifer's headaches and pain. Case Presentation: Anterior Veneer Replacement

Today's patient came to us with a set of porcelain veneers on their two front teeth, where one had become cracked along the gum line. Defects in this location are particularly problematic, as they create an easy place for bacteria to enter the tooth and are cosmetically unpleasing. After discussing options with the patient, we decided to replace both veneers with new Emax lithium disilicate units. Lithium disilicate is an engineered dental material that is about five or six times stronger than porcelain, along with having more "natural" cosmetic properties. By replacing both veneers, we were able to repair the defect, add length to the teeth (the original veneers were short of optimal) and create overall more natural contours. Case Presentation: Anterior Smile Perfection

Another Great veneer case from Dr. Kari Ann Hong! 

This patient came to our office wanting to restore the esthetics and proportions of their smile. The maxillary anterior teeth appeared jagged and uneven due to a combination of grinding and dietary acid wear. The unnatural sharpness in tooth structure lends itself to an older appearing or "worn down" smile. Additionally, old composite (tooth colored) fillings had begun to show contrast against the natural tooth structure. As teeth age, they tend to pick up stains and wear at a rate different from composites. Over decades, these small differences can become very evident, particularly in patients who consume lots of dark liquids (coffee, soda, red wine, etc.).

In recreating this smile, Dr. Kari used a series of eight lithium disilicate veneers. These restorations give the teeth a uniform shape and color, while mimicking the natural opacity of dental enamel. In just two weeks, this smile looks years younger! Case Presentation: Special Veneer Restoration

This case is a great example of the more complex esthetic procedures performed at our office. Today's patient came to us with discoloration and decalcification on their anterior teeth. Additionally, their left lateral incisor (the lightest colored tooth in the before pictures) had an existing crown over a root canal. It had a poor long term prognosis and could not endure any more trauma. With this in mind, Dr. Kari created a set of five veneers that restored symmetry to the patient's teeth while incorporating the esthetics of the previous crown. The result is a more natural and youthful smile that does not compromise the existing dental work. A true win-win!  

Dental Questions: What are my options in improving the esthetics of a single tooth?

This new tooth was restored using only bonding and disking. What a change!

Teeth can become cosmetically damaged through a number of different means; from chipping to fractures, staining, decay or even as a result of problems in development. Likewise, our office offers a variety of ways to restore your teeth and make them appear natural in your mouth once again.

As with any procedure, we start the process by making sure the tooth in question is sill alive and healthy. There is no sense in restoring a tooth with serious underlying problems. Secondly, we assess the extent of the damage. For minor chips and blemishes, a combination of bonding and “disking” may be the perfect fix. This process is virtually drill-less and conserves as much tooth structure as possible. You may be surprised to see what we can achieve without a crown or veneer!

For teeth with larger blemishes, or those missing a substantial amount of structure, we will often elect to utilize a crown or veneer. Cosmetic veneers are usually placed on anterior (front) teeth to modify their appearance. Since they are cemented to only one surface, they do not add any additional structural integrity. In other words, the underlying tooth must be in good condition to receive a veneer. In contrast, crowns can be used to improve the cosmetics of a tooth that has lost a significant amount of structure.  Since crowns wrap around the tooth, they can restore shape in every dimension while adding strength and rigidity to the underlying enamel and dentin.
